Clinicians write notes after hours. Dictation tools give you a transcript, which is not a note, and most of them send raw patient audio to a third party.
Audio is transcribed, speakers are separated, then named entities are detected and replaced with placeholders before any text reaches the model. The note is generated against redacted text and the real identifiers are rehydrated locally afterwards.
- A guardrail classifies the recording first and fails closed: a non-clinical recording never becomes a clinical note.
- PHI placeholders such as [NAME_1] and [DATE] are preserved verbatim through generation.
- Arabic and English are both handled, including Arabic named-entity recognition.
